Although we accept self-referrals, we prefer to receive the additional clinical and social care information that a healthcare professional working with the client can provide. If you are referring a client or patient you are working with, please email dhc.wheelchair.services@nhs.net. A digital form is under development as part of self-referral. All NHS wheelchair services have eligibility criteria to ensure an equitable service.

If your patient/client does not meet NHS criteria, please see signposting information below.

The wheelchair and associated accessories remain the property of the NHS and are on loan to the individual for as long as that person meets eligibility criteria and is using the wheelchair in accordance with NHS conditions of loan (see below); the exception to this is wheelchairs provided through third party personal wheelchair budget.

Supply of the wheelchair is based on clinical reasoning, as required for the use of a medical device.

Supply is always in accordance with medical devices regulations (MHRA).

Wheelchairs can be loaned entirely through the NHS with no additional funding from the individual; however, elements of the wheelchair can be personalised through the use of the personal wheelchair budget, please ask for more information.

When a wheelchair is no longer in use, it must be returned to the service (collection can be arranged).

Wheelchairs that are returned to the service are refurbished (if suitable) and re-issued for NHS loan.
